Report Types

Introduction

SpokeChoice contains many different types of reports. In order to use these reports it is important to add them to your companies profile. To learn how to do this click here and you must have admin access to access backoffice

The types of reports that are available are as follows

Report Name Description
Active & Inactive Clients Report allows specifying all information about a client. Inactive or active with all necessary fields
Active & Inactive Providers
Report allows specifying all information about a client. Inactive or active with a few fields
Administration roles
Report shows all users that have administrative roles and what level, superuser, admin or backoffice
All Doctors Report will show all the doctors you have in SpokeChoice
Authorization Expiring By
Report shows all authorizations expiring by percentage or hours
Authorization expiring By Date
Report shows all authorizations expiring by a certain date.
Billing Claims
Report shows lots of information about billing, including total charge, route number, place of service, TPL Amounts, Provider information, referring doctor, AHCCCS ID, Billing Code, Provider name, Client Name, Total Units, Next payer, paid amount and date, MCID's start and stop time, and supervisor name
Billing Rates
Report lists all the billing rates on every service code
CEU Information Report will show who needs more CEU Units by a given date and also show totals for the given period
Check Maximum Daily Hours
Report will show who is over maximum work week hours by definition of the ISP Maximums.
Claim Overlaps
Report will show which claims have overlap of two different providers. This happens when the first provider neglects to clock out or didn't have internet availability at the time of clock out.
Client Assignments
Report will show which clients are assigned to which Providers
Client Authorization Balances
Report shows simple current Authorizations balances. Will also show maximum work week hours against allowed hours
Client Expiring ISP
Report shows the which ISP's are expiring by a given date
Client Policy Information
Report shows all active clients insurance information
DDD Billing Report
Report will show total Units, service Rates, Amount Paid, start / end times, approved, submitted, unapproved, paid, service code and provider ONLY for DDD claims.
DDD OLCR Report
OLCR Report that can be uploaded to FOCUS OLCR
Denied Claims
Report will show all denied claims for a given date range
Documents
Report shows all sorts of data related to the document Vaults documents
Drivers data
Report will show all providers and whether they are an approved Driver. It also will add any custom fields that have been added to the provider data page.
EVV Client Info Check
Report will show any errors that need to be fixed for EVV related data for clients
EVV Employee Info Check
Report will show any errors that need to be fixed for EVV related data for providers
EVV History and Proof
Report can be used for an audit to show the necessary EVV related data. Has a schedule, manually entered, in and out EVV data captured, Longitude / latitude data and Changed by provider
EVV Report card
Report will grade a provider based on clocking in and out on time. 81-100% = A, 61-80% = B, 51-60% = C, 31-50% = D and 0-30% = F
Employee Admin Service Totals
Report will show the total hours for any Admin (payroll only) provider recorded claims
Employee Payroll Amount
Report will show all employees total Worked amount for a given date range grouped by service Code
Employee Total Payroll Amount
Report will show the employees total worked amount and total gross pay for a given date range.
Employee Work Week Hours
Report will show all active employees work week amounts
Employee claim/admin items
Report will show employee total amounts for a given date range and whether it is paid or not.
Employee payrates
Report will show all employee Pay rates. Make sure only Superusers have access to this report.
Employees on Payroll Hold
Report will show all employees that are on payroll hold. Use this to determine why payroll for an employee might not be exporting.
Expiring Certifications
Report will show expiring certifications and dates.
Field Supervisor/Client Assignments
Report will show which Field supervisors are assigned to which clients
Field Supervisor/Employee Assignments
Report will show which Field supervisors are assigned to which providers
Fingerprint application / card Check
Report will show finger print application and finger print numbers and the effective and expiration dates
Government Exclusions
Report will show OIG information that has been checked.
Group Home and Center Profit
Report will show profit for group homes and DTA centers
Guardian Assignments
Report will list which Clients are associated to which Guardians.
Last Day of service
Report will show the last date of service for all providers
Live-in Providers
Report will list only the providers that are designated as a live-in with the guardian / client Assignment
Manual Authorizations
Report will list all Manual Authorizations. Use this to remove manual authorizations that will supersede Payer authorizations.
Missing Paper Timecard
Report will list missing hardcopy timecards if wetink is used on the guardian / client assignment
Missing Passwords
Report will show any identity that is missing a password. Helpful will determining if a provider or guardian has neglected to set up their password in a timely manner.
Missing three letter code
Report will show any three digit DDD codes that are missing. This is important to check to ensure authorizations from DDD accurately are assigned.
Outstanding Insurance Claims
Report will show all outstanding Insurance claims.
Payroll Cash Requirement
Report will show the payroll cash amount required for a given date range of unpaid payroll.
Provider Guardians
Report will list only the Live-in providers.
Provider Information
Report will give detail information about an provider including 1099 / W2 and who is configured to a department
Providers billing HAH at Midnight
Report will show who is billing HAH over night which is not allowed by DDD.
Providers over 16 hours
Report will show all providers that are working more than 16 hours. Run this prior to submitting billing to see if you will get denied for more than 16 hours.
SOAP notes needing Supervisor signatures
Report will show which SOAP notes need supervisor signatures.
Schedule greater than 24 hours
Report will show who is scheduled more than 24 hours
Service Code Pointers
Report will show all service code pointers and how they are mapped to each payer per client
Service Codes
Report will show all information about all service codes.
Service Performance
Report is a profit statement that helps to show client, provider and service code gross profit with a given date range
Service Waivers
Report will show all service TPL waivers that are entered for clients. Use this if you can not figure out why a certain payer is skipped or claims are not going out.
Sick Time Balance
Report will show all providers sick time balance
Subscribed to Notifications
Report will show who is subscribed to message notifications. Use this to determine who has accepted SpokeChoice notifications and can receive a message directly to their device.
Time Card Errors
Report will list any claims that have errors
Timecard Signature Check
Report will show if there are missing Timecards for manual wetink signatures
Total Billed Units
Report will show total Units billed for clients for a given date range
Unpaid Claims
Report will show all unpaid claims
Upcoming and needing Visits
Report shows all clients / providers that need a monitor visit for Attendant care (ATC)
User Activity
Report lists all users that have signed in for today.
Waiting Guardian Signatures
Report lists all guardians that need to sign timecard
