Setting-Up a Designee
With AHCCCS policy addition of assigning a designee for a member, we would like to show you how Spokechoice can help with the creation of the new account and storing of the Designee Attestation form
Creation of Account:
We suggestion you create your designee an account as a Guardian, even if they are not a legal guardian.

You will want to assign the designee to the member in the Guardian Assignment tab

You can specify in the guardian assignment the relationship between the member and the designee

Designee Attestation Form:
If you would like to store your Desginee Attestation form within spokechoice you will want to add a new client expiration.
Navigate to Back office-Company Options- Expiration Type

Choose +new in the right corner

Input an Expiration Name and who the expiration is for . Choose Add

Next you need to add the new document type. Navigate to Back office-Document Vault

Choose New

Input
1. Document Type Name
2. Employee Type
3. Requires Client
4. Requires Expiration
5. Expiration Type: Choose Designee Attestation

Choose save
You can now upload the designee form to the clients field.
Upload of Designee Attestation Form
Navigate to Document Vault
Choose client- Choose +New Document

Choose the Designee Attestation Doc Type, Choose the Guardian/Designee who has been assigned to the member, choose the member, input the expiration date, check review
Now upload the form , once uploaded the screen will close
