1. This is whether this guardian will electronically sign. If they will not sign then do not check this.

2. If they are allowed to sign for there own provided services check this box. If they can not sign for there own services do not check this. The client will need another guardian created/assigned in order to have this guardians timecards approved.

3. If the electronic signature required is check you will need to indicate the start of the signature date. Make sure and indicate the actual time needed and do not go back in time where you already have wet ink signatures. If you do then the guardian will see more needed signatures than necessary.

4. This is necessary for EVV communication. You must indicate what type of a guardian this is.

Note: If you want to make a client their own guardian you must create a new identity for the individual. There is no way to add a provider role to the client. The system will warn you about adding a duplicate individual, that is ok proceed and give that individual the provider role. If you are using iSolved you can also just add the individual into payroll and refresh as it will add the provider identity and role. You can also give the guardian role to this individual on the provider identity if they will be signing for themselves. When assigning the provider/guardian to the client you will note the name will be the same.
