DDD has specific information that must be updated.

The following information is imperative for both synchronizing Authorizations and creating your billing file.

• EIN - Your federal tax identification number
• Four Letter Billing code - Used to identify your company when billing
• AZ AHCCCS ID - The number from AHCCCS for your agency identification
• AZ AHCCCS ID - Sometimes an agency might have an addition number just for Therapy.
• Contract Number - Your contract number for DDD. This should likely only be a 6 digit number. Do not add the year that contract was issued (front of the number)
• Company NPI - Licensed therapy company typically only have an NPI number.
• FOCUS Login - Your Login identification for FOCUS (Note: it is best to have a login that is just for SpokeChoice to avoid having to change your password)
• FOCUS Password - The FOCUS password for the above login
• FOCUS FTP Login / password - The specific login and password for DDD FTP site so you can upload your progress reports.

To enter or update these numbers navigate to Backoffice - Company Options.

Fill out the information and hit save.

WARNING: You should only be updating this form if you know what you are doing. Updating this with any misinformation could cause problems during billing or synchronizing Authorizations.
